Counter-Notice Procedure

  1. Your name, address, phone number, and email address.
  2. Identification of the material that has been removed and the URL where it previously appeared.
  3. A statement under penalty of perjury that you have a good faith belief that the material was removed as a result of a mistake or misidentification.
  4. A statement that you consent to the jurisdiction of the federal district court in your area, and that you will accept service of process from the original complainant or their agent.
  5. Your signature (electronic or physical).
